Community Health Needs Assessment

Community Benefit Grant Program

Inland requests proposals to improve community health

As a member of EMHS, Inland Hospital is committed to improving the health of the communities we serve. Our Community Benefit grant program is designed to help address priority health issues facing local communities by expanding partnerships and use of evidence-informed prevention practices. Inland Hospital is seeking proposals from non-profit organizations that measurably impact one of these priority health issues as identified in our 2016 Shared Community Health Needs Assessment (CHNA):
  • obesity
  • substance abuse/opioid harm reduction
  • mental health
  • poverty/economic opportunity
  • healthy food access
Please refer to Inland's Community Health Improvement Plan 2017-2019 goals and objectives.

Approximately four awards will be made; applicants may request up to $5,000 for projects to be completed by December 30, 2018.

Successful applicants will demonstrate the ability to carry out implementation projects which meet the following criteria:
  1. Address a priority need and target a priority population
  2. Reach significant numbers of people in the priority population
  3. Use evidence-informed approaches that are likely to achieve results
  4. Define measurable objectives and how results will be measured
  5. Leverage the strength of partners to implement and sustain the effort
We recommend reviewing the Shared CHNA at inlandhospital.org and applying its data to proposals. We also encourage you to visit countyhealthrankings.org to find evidence-informed strategies that make the most impact. Finalists may be invited to present their proposals to a selection committee.

Typically, we do not provide funding for the following:
  • Organizations outside of the Greater Waterville service area of Inland Hospital
  • Individuals
  • Individual scholarships or travel-related expenses
  • Religious organizations
  • Political organizations
  • Sponsorships of athletic teams or pageants
  • Advertising in yearbooks or sports calendars
  • Individual youth clubs
  • Conventions
  • Labor groups
  • Social clubs
The deadline for completed applications is Monday, February 11, 2018. If you have questions, please contact the Inland Hospital Community Relations department at 861-3293 or inlandcr@emhs.org. Thank you.

Please note: Please save your answers in a Word Document from which you can copy and paste into the online application boxes once you are ready to submit your application. The reason is if you input your answers directly into the online application and have to step away from your desk or for some reason are unable to finish, your responses will not be saved because the responses have not been submitted, thus are temporary and will be deleted once you close out of the application. Once you have submitted the form, you will received an email confirmation. Thank you.
