Inpatient Hospital-acquired Pressure Ulcer Grade II or greater - Graph and Details
Inpatient pressure ulcers are determined through a prevalence study. A team of nurses trained in the assessment of skin integrity reviews the chart of every patient in the hosptial on a designated day. The nurses then examine the patient to determine the presence of pressure ulcer(s). If the pressure ulcer is not documented on admission or has deteriorated during hospitalization, then a hospital-acquired pressure ulcer is identified. The desired performance is low.
